Roles & Responsibilities
- Provide compassionate nursing care to patients in the Uptown Health/Rehab area
- Administer medications and treatments accurately and document thoroughly
- Collaborate with interdisciplinary teams to create and follow individualized care plans
- Monitor patient progress and report changes in condition promptly
- Support patients and families with education, discharge planning, and resources
- Maintain accurate clinical records and follow facility policies and regulatory standards
Qualifications
- Valid LPN or RN license (eligible to practice in New Mexico preferred)
- Current Basic Life Support (BLS) certification or willingness to obtain
- Clinical experience in long-term care, rehabilitation, or similar settings preferred
- Strong patient assessment, communication, and documentation skills
- Ability to work 12+ hour day shifts and manage patient caseload safely
Education
- LPN: Practical nursing diploma or certificate
- RN: Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN)
Knowledge & Skills
- Knowledge of medication administration and common nursing procedures
- Skilled in patient assessment, wound care, and mobility assistance
- Familiarity with electronic health records and accurate charting
- Excellent teamwork, prioritization, and time management skills
- Compassionate bedside manner and strong patient/family communication
