Roles & Responsibilities

• Provide compassionate direct patient care to residents in a skilled nursing setting
• Administer medications and treatments safely and accurately
• Monitor and record vital signs and changes in patient condition
• Document care, progress, and interventions in patient records with clarity
• Collaborate with nursing staff and multidisciplinary team to support individualized care plans
• Support residents and families with clear communication and empathy
• Follow infection control protocols and maintain a safe environment for residents

Qualifications

• Current Licensed Practical Nurse (LPN) license — Pennsylvania preferred or eligible
• CPR/BLS certification (or willing to obtain upon hire)
• Comfortable working 8-hour shift schedules and providing hands-on resident care
• Ability to pass background check and meet facility onboarding requirements
• Previous experience in skilled nursing or long-term care is preferred but new grads with strong clinical skills are welcome

Education

• Completion of an accredited LPN/Vocational Nursing program
• High school diploma or equivalent

Knowledge & Skills

• Clinical assessment and basic nursing skills for adult and geriatric populations
• Medication administration and documentation best practices
• Familiarity with electronic health records and charting
• Strong communication and teamwork skills
• Knowledge of infection control and safety procedures
