Roles & Responsibilities
- Provide compassionate direct patient care consistent with LPN scope of practice
- Administer medications and document accurately in the medical record
- Monitor patient status, report changes to the RN or provider, and escalate concerns promptly
- Assist with activities of daily living and ensure patient comfort and safety
- Collaborate with interdisciplinary team members to support individualized care plans
- Maintain clear, timely charting and uphold confidentiality and compliance standards
- Support admissions, discharges, and patient transfers as needed to ensure smooth operations
Qualifications
- Current, active LPN license in Pennsylvania (or compact/reciprocal as applicable)
- CPR/BLS certification required
- Previous LPN experience in a long-term care or similar clinical setting preferred
- Ability to pass background check and meet facility health/vaccination requirements
- Strong communication skills and the ability to work effectively as part of a team
Education
- LPN diploma or certificate from an accredited program
- High school diploma or GED
Knowledge & Skills
- Skilled in medication administration and basic wound care
- Comfortable with patient assessments and documenting changes
- Familiarity with electronic medical records (EMR) or charting systems
- Good clinical judgment, time management, and prioritization skills
- Demonstrated empathy and strong patient-centered care focus
