Roles & Responsibilities

Provide high-quality nursing care for clinic patients within the scope of LPN licensure, ensuring safety and compassion.
Act as the primary liaison between patients, clinical staff and providers to coordinate excellent care.
Perform patient triage, obtain and interpret vital signs, and assess patients to determine appropriate next steps.
Manage medication refills and complete necessary prior authorizations to keep care moving smoothly.
Give direction to CNAs and Schedulers and collaborate closely with RNs, Providers, Coordinators and Clinical Managers.
Document patient interactions and follow current state and federal licensure and continuing education standards.
Support the care team by stepping in where needed to maintain clinic flow and positive patient experiences.

Qualifications

Current State LPN license required
BLS (Basic Life Support) certification required
Graduate of an accredited nursing program
Comfortable performing triage, taking vital signs, and conducting patient assessments
Experience with medication refills and prior authorization processes
Strong patient communication skills and ability to collaborate with interdisciplinary teams
Willingness to work Monday–Friday with some weekend availability

Education

Graduate of a Nursing Program (LPN program completion required)
