Roles & Responsibilities

• Assist residents with activities of daily living — bathing, dressing, toileting, and feeding — with respect and dignity
• Monitor and record vital signs and promptly report changes to nursing staff
• Support rehabilitation goals by assisting with mobility, transfers, and exercise programs
• Maintain accurate, timely documentation and follow assigned care plans
• Follow infection control, safety protocols, and facility policies
• Communicate clearly and compassionately with residents, families, and the care team
• Contribute to a positive, person-centered environment that promotes independence

Qualifications

• Current GNA certification or completion of an accredited GNA program
• CPR/BLS certification required or obtained upon hire
• Ability to work overnight shifts and flexible per diem schedules
• Comfortable assisting with transfers and mobility; strong focus on safe body mechanics
• Excellent communication skills and a team-first attitude
• Ability to pass background check, drug screen, and meet facility health requirements

Education

• High school diploma or GED preferred
• Completion of an accredited GNA program or equivalent

Knowledge & Skills

• Understanding of geriatric care principles and common age-related conditions
• Hands-on experience with ADLs and mobility assistance
• Basic vital sign measurement and documentation skills
• Good time management and the ability to prioritize resident needs
• Compassion, patience, and strong interpersonal skills
• Ability to work collaboratively within a rehab interdisciplinary team
