Roles & Responsibilities

• Provide high-quality nursing care to residents including assessments, monitoring, and documentation
• Administer medications and treatments safely and in accordance with orders
• Perform wound care and other skilled procedures within scope of practice
• Monitor vital signs and report changes in resident condition promptly
• Collaborate with CNAs, therapists, physicians, and family members to create and follow individualized care plans
• Maintain accurate clinical documentation in the electronic medical record
• Promote a safe, respectful, and dignified environment for residents
• Support discharge planning and effective communication during shift handoffs

Qualifications

• Active LPN license in Massachusetts or Active RN license in Massachusetts (must be in good standing)
• Current Basic Life Support (BLS) certification
• Previous experience in long-term care, rehab, or skilled nursing preferred but motivated newer grads are welcome
• Strong clinical assessment and medication administration skills
• Clear background checks and ability to meet facility onboarding requirements

Education

• LPN: Graduate of an accredited LPN/Vocational Nursing program
• RN: Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN)

Knowledge & Skills

• Skilled in medication administration and clinical documentation
• Knowledge of wound care, infection control, and resident safety practices
• Comfortable using electronic medical records and charting accurately
• Excellent communication and teamwork skills with residents and families
• Ability to prioritize tasks in a fast-paced skilled nursing environment
