Roles & Responsibilities
- Provide high-quality nursing care to residents including assessments, monitoring, and documentation
- Administer medications and treatments safely and in accordance with orders
- Perform wound care and other skilled procedures within scope of practice
- Monitor vital signs and report changes in resident condition promptly
- Collaborate with CNAs, therapists, physicians, and family members to create and follow individualized care plans
- Maintain accurate clinical documentation in the electronic medical record
- Promote a safe, respectful, and dignified environment for residents
- Support discharge planning and effective communication during shift handoffs
Qualifications
- Active LPN license in Massachusetts or Active RN license in Massachusetts (must be in good standing)
- Current Basic Life Support (BLS) certification
- Previous experience in long-term care, rehab, or skilled nursing preferred but motivated newer grads are welcome
- Strong clinical assessment and medication administration skills
- Clear background checks and ability to meet facility onboarding requirements
Education
- LPN: Graduate of an accredited LPN/Vocational Nursing program
- RN: Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN)
Knowledge & Skills
- Skilled in medication administration and clinical documentation
- Knowledge of wound care, infection control, and resident safety practices
- Comfortable using electronic medical records and charting accurately
- Excellent communication and teamwork skills with residents and families
- Ability to prioritize tasks in a fast-paced skilled nursing environment
