Shift Type: Varied 8-hour shifts, including weekends and holidays; rotating schedule with no guaranteed shift or hours (candidates must be flexible)
Hours per Week: 40
Candidate Pay Rate: $49 per hour

Roles & Responsibilities
- Perform thorough patient assessments to identify changes in condition and care needs
- Administer medications and treatments accurately and safely
- Provide specialized care including tracheostomy, gastrostomy tube, and urinary catheter management
- Deliver wound care and Wound VAC management with attention to detail
- Carry out respiratory treatments such as nebulized therapy and CPAP support
- Assist residents with activities of daily living to preserve dignity and independence
- Coordinate admissions and discharges, ensuring smooth transitions of care
- Communicate pertinent clinical information promptly to MD/NP and interdisciplinary team
- Document care clearly and maintain infection control and safety standards
Qualifications
- Current Licensed Practical Nurse (LPN) licensure (required)
- Experience in long term care or similar settings preferred
- Proof of influenza vaccination required prior to contract start date
- Employee health clearance required before starting assignment
- Flexible availability with willingness to work weekends and holidays
- Strong communication skills and ability to collaborate with MD/NP and care team
Education
- Graduation from an accredited practical nursing program or equivalent
- Current LPN licensure required
Knowledge & Skills
- Medication administration and treatment administration best practices
- Patient assessment and clinical observation skills
- Gastrostomy tube care and urinary catheter care
- Tracheostomy care and respiratory treatments (nebulized therapy, CPAP)
- Wound care including Wound VAC management
- ADL assistance and support for activities of daily living
- Admissions and discharge processes in long term care
- Clear documentation and timely clinical communication
