Roles & Responsibilities

• Conduct screening, assessment, diagnosis, and treatment planning for patients from birth through elder
• Provide brief direct therapy and continuing care, documenting progress and treatment plans
• Lead and facilitate discharge planning and patient care conferences, including ALF/transitional placements
• Coordinate care with hospital teams and community partners to ensure smooth transitions and optimal outcomes
• Advocate for clients, assist with complex matters (e.g., Power of Attorney), and support skill-building
• Maintain accurate, timely, and compliant documentation for clinical care and hospital billing
• Support quality management initiatives, attend trainings, and help integrate best practices into care delivery
• Assist with assigned billing duties and provider enrollment documentation as requested

Qualifications

• Master's degree in Social Work, Psychology, Counseling, or a related discipline from an accredited institution
• Licensed or in the process of becoming licensed as an LCSW, LPC, or LMFT
• Valid State of Alaska driver’s license or ability to obtain within 6 months of hire
• Must be 21 years of age
• Clinical Competency Assessment (SEARHC) required within 3 months of hire and every three years
• Experience with discharge planning, case management, care coordination, and crisis intervention
• Strong documentation skills meeting state, federal, accreditation, and hospital billing standards

Education

• Master's Degree in Social Work, Psychology, Counseling, or related discipline (accredited institution)

Knowledge & Skills

• Discharge planning and care coordination across the continuum of care
• Clinical assessment, screening, diagnosis, and brief therapy
• Crisis intervention techniques and interviewing skills
• Familiarity with hospital billing documentation requirements and provider enrollment
• Multidisciplinary team collaboration and tribal health / cultural wellness awareness
• Case management, documentation compliance, and quality management practices
