Roles & Responsibilities

• Coordinate patient care across the continuum as the central point of contact for members
• Conduct thorough clinical assessments and develop individualized case management plans
• Collaborate with interdisciplinary teams to ensure safe transitions of care
• Advocate for members to secure appropriate resources, authorizations and services
• Monitor utilization and document outcomes to support quality and cost-effective care
• Communicate clearly with members and families to educate and empower decision making
• Maintain accurate, timely clinical documentation and meet program standards

Qualifications

• Active Registered Nurse (RN) license in California (required)
• Clinical experience in case management, utilization management or related nursing role
• Strong clinical assessment skills and care coordination experience
• Valid basic life support (BLS) certification or willingness to obtain
• Comfortable working with interdisciplinary teams and community resources
• Excellent verbal and written communication skills

Education

• Associate's or Bachelor of Science in Nursing (preferred)

Knowledge & Skills

• Care coordination and discharge planning best practices
• Member advocacy and resource navigation
• Clinical assessment and care planning
• Familiarity with utilization review and case management workflows
• Strong documentation skills and attention to detail
• Ability to collaborate effectively with clinical and non-clinical stakeholders
