Roles & Responsibilities

• Perform thorough patient assessments to monitor resident status and needs
• Administer medications and ordered treatments safely and accurately
• Provide wound VAC care and specialized wound treatments
• Manage gastrostomy tube and urinary catheter care with attention to infection prevention
• Deliver nebulized treatments and CPAP support when indicated
• Assist residents with activities of daily living to promote dignity and comfort
• Coordinate and promptly communicate pertinent clinical information to MD/NP
• Conduct admissions and discharges with accurate documentation and patient education
• Maintain clear, timely documentation of care and observations
• Participate in employee health requirements, including flu vaccination or approved exemption

Qualifications

• Current and active LPN license in the required jurisdiction
• Experience working in a Long Term Care (LTC) setting
• Demonstrated competency in medication administration and patient assessments
• Hands-on experience with wound VAC care, gastrostomy tube care, and urinary catheter care
• Comfortable providing nebulized treatments and CPAP care
• Strong communication skills for collaborating with MDs, NPs, and interdisciplinary teams
• Proof of flu vaccination or facility-approved medical or religious exemption prior to start

Education

• Graduate of an accredited Practical/Vocational Nursing program and active LPN license
