Roles & Responsibilities

• Evaluate and treat skilled nursing facility (SNF) patients using evidence-based OT approaches
• Collaborate closely with the interdisciplinary team to create cohesive care plans
• Complete thorough documentation in Net Health per facility standards
• Manage caseloads efficiently with strong time-management and prioritization
• Provide compassionate, patient-centered care that promotes independence and quality of life
• Float to a nearby sister facility when needed to support continuity of care

Qualifications

• Active CA OT License (or license in process under the 60-day rule)
• SNF experience preferred; new graduates welcome — mentorship available
• Strong documentation skills, particularly with Net Health
• Excellent time-management and interpersonal skills
• Flexible, collaborative mindset and ability to work well in an interdisciplinary team

Knowledge & Skills

• Occupational Therapy evaluation and treatment planning
• Skilled nursing facility (SNF) workflows and regulations
• Proficient documentation practices and Net Health experience
• Interdisciplinary collaboration and communication
• Patient-centered care and rehabilitation strategies
• Ability to adapt and float between nearby facilities
