Roles & Responsibilities

• Coordinate comprehensive case management for assigned patients, ensuring continuity of care
• Assess patient needs, develop care plans, and collaborate with interdisciplinary teams
• Facilitate safe and timely transitions of care from hospital to home or other settings
• Advocate for patients and families to secure appropriate resources and supports
• Document care plans, progress, and outcomes clearly in the medical record
• Communicate effectively with physicians, social workers, and community providers to optimize patient outcomes

Qualifications

• Active Registered Nurse (RN) license in California or compact RN with CA eligibility
• Experience in case management or care coordination (experience in utilization review a plus)
• Current BLS certification; ACLS or certifications relevant to case management are a plus
• Strong clinical assessment, triage, and discharge planning skills
• Excellent communication and documentation abilities with attention to detail
• Ability to work independently while collaborating with an interdisciplinary team

Education

• Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) preferred

Knowledge & Skills

• Knowledge of case management principles and care coordination processes
• Familiarity with electronic medical records and documentation best practices
• Skilled in patient advocacy, resource identification, and community referrals
• Strong time management and prioritization skills in a fast-paced environment
• Ability to interpret clinical information and translate it into actionable care plans
