Roles & Responsibilities

Conduct comprehensive start of care (SOC) assessments for new home health patients
Perform resumption of care (ROC) and recertification visits to ensure continuity
Manage daily visits and coordinate care plans with interdisciplinary teams
Educate patients and families on treatment plans and home care procedures
Document patient progress in Homecare Homebase (HCHB) accurately and timely

Qualifications

Active Registered Nurse (RN) license in California
At least one year of Home Health experience within the last two years
Current BLS certification from the American Heart Association
Strong preference for experience with Homecare Homebase (HCHB) EMR

Knowledge & Skills

Expertise in home health nursing including start of care, resumption, and recertification
Proficiency in HCHB charting and documentation
Excellent clinical assessment and critical thinking skills
Ability to manage a patient caseload with productivity expectations of 3-4 visits daily
Strong communication and patient education abilities
