Shift Type: Monday-Friday, Days, possible weekend shifts

Hours per Week: 36

Candidate Pay Rate: $70 per hour

Roles & Responsibilities

Provide comprehensive nursing care to patients of all ages in their homes
Conduct initial admissions, evaluations, and develop individualized care plans
Coordinate care with a multidisciplinary team of home health professionals
Monitor patient progress and adjust care plans as needed
Educate patients and families on disease management and home care techniques
Document all patient care activities accurately and in a timely manner

Qualifications

Current and active Vermont RN license required
Minimum 1 year of recent clinical nursing experience
Home health experience strongly preferred
Valid driver's license and reliable transportation

Knowledge & Skills

Strong clinical assessment and critical thinking skills
Excellent communication and interpersonal abilities
Proficient in care planning and documentation
Ability to work independently and as part of a team
Knowledge of home health regulations and best practices
