Shift Type: Monday-Friday, 8:00 AM - 4:30 PM

Hours per Week: 35

Candidate Pay Rate: $63 per hour

Roles & Responsibilities

Provide expert physical therapy assessments and treatments to help patients regain mobility and independence
Develop and implement individualized care plans tailored to each patient's goals
Collaborate with a multidisciplinary team to ensure comprehensive patient care
Educate patients and families on therapeutic exercises and home safety
Document progress accurately and maintain compliance with facility standards

Qualifications

Current Physical Therapist license in New Hampshire (or compact privilege)
Graduation from an accredited physical therapy program
BLS certification from the American Heart Association
At least 1 year of recent experience in a skilled nursing or rehab setting preferred

Knowledge & Skills

Strong clinical assessment and treatment planning skills
Excellent communication and patient education abilities
Ability to work independently and as part of a team
Proficient with electronic medical records (EMR) systems
