Roles & Responsibilities

• Provide skilled physical therapy evaluation and treatment to residents in a skilled nursing setting
• Develop individualized, goal-oriented treatment plans and update them based on patient progress
• Document assessments, interventions, and progress clearly and timely in the patient record
• Collaborate with nurses, occupational therapists, speech therapists, and physicians to coordinate care
• Educate patients and families on therapeutic exercises, mobility strategies, and fall prevention
• Monitor patient safety during treatments and modify interventions to match tolerance and goals
• Participate in interdisciplinary team meetings and contribute to discharge planning

Qualifications

• Current Physical Therapist license in Pennsylvania or eligibility to obtain one
• Degree in Physical Therapy (DPT preferred) or equivalent clinical training
• Experience or comfort working in a skilled nursing/rehab environment preferred
• Basic Life Support (BLS) / CPR certification (or willingness to obtain)
• Excellent documentation skills and patient-centered communication
• Ability to work Monday-Friday daytime schedule reliably

Education

• Doctor of Physical Therapy (DPT) preferred; Master's or Bachelor's in Physical Therapy acceptable where applicable
• Relevant state licensure required

Knowledge & Skills

• Skilled in assessment of mobility, strength, balance, and functional status
• Proficient with therapeutic exercise, gait training, transfers, and assistive device training
• Knowledge of SNF/long-term care rehab best practices and regulatory documentation
• Strong clinical reasoning and ability to set measurable functional goals
• Comfortable working collaboratively within an interdisciplinary team
