Roles & Responsibilities

Assess and treat patients with movement dysfunction and pain to restore function and independence.
Develop and implement individualized treatment plans focused on mobility improvement and fall risk reduction.
Document evaluations, progress notes, daily notes, discharges, and communications with insurers accurately and promptly.
Coordinate with other providers, rehab staff, and insurance companies to support smooth care transitions and approvals.
Assist with discharge planning to optimize patient outcomes and safe transitions home or to next level of care.
Support clinic operations by helping maintain clean treatment rooms and assisting with light laundry and facility upkeep.

Qualifications

Current license to practice as a Physical Therapist in Wisconsin or eligibility to obtain one.
Doctor of Physical Therapy (DPT) or Master’s in Physical Therapy strongly preferred.
Strong documentation skills and comfort with writing evaluations, progress notes, and discharge summaries.
Excellent communication skills for coordinating with providers, patients, families, and insurance representatives.
Ability to perform patient transfers and mobility tasks as required and to follow safety protocols.

Education

Doctor of Physical Therapy (DPT) preferred; Master’s in Physical Therapy acceptable
Active PT licensure in Wisconsin or eligibility to obtain licensure

Fall risk assessment and reduction techniques
Mobility improvement and independence-focused care
Discharge planning and care coordination
Thorough documentation and progress note preparation
Effective communication with providers and insurance companies
