Roles & Responsibilities

Provide compassionate physical therapy services to patients in their homes across Yolo County
Assess and evaluate each patient's therapeutic needs and develop personalized care plans
Guide patients and families through rehabilitation exercises and therapeutic appliance use
Monitor progress and communicate changes in condition to physicians promptly
Collaborate with home health aides and caregivers to ensure consistent care
Document clinical summaries and maintain accurate patient records
Participate in case conferences to coordinate comprehensive patient care

Qualifications

Minimum 2 years experience as a Physical Therapist in acute, home care, or rehab settings
Current BLS certification required
Strong oral and written communication skills
Excellent organizational abilities
Valid driver's license and reliable transportation for home visits

Knowledge & Skills

Physical therapy assessment and treatment planning
Home environment evaluation and safety assessment
Patient and family education techniques
Prosthetic device training
Medicare and Medi-Cal compliance knowledge
Title 22 and OSHA regulations understanding
