Roles & Responsibilities

• Evaluate patients to determine physical therapy needs and create individualized treatment plans
• Deliver evidence-based treatment programs in patients' homes following agency policies
• Re-evaluate and adjust treatment plans based on patient progress and goals
• Coordinate and communicate care activities with the interdisciplinary team to ensure seamless care
• Document evaluations, goals, treatments, and outcomes clearly and timely
• Teach patients and caregivers the rationale and techniques for home programs to support independence

Qualifications

• Licensed Physical Therapist in Vermont and New Hampshire, or hold a compact PT license (unencumbered)
• Graduate of an accredited school in Physical Therapy
• Home health experience preferred; at least one year in a home health PT setting is a plus
• Strong documentation and communication skills appropriate for interdisciplinary home care

Education

• Graduate of an accredited school in Physical Therapy (degree or certificate as required by licensure)

Knowledge & Skills

• Skilled in patient evaluation, treatment planning, and rehabilitation techniques
• Knowledge of home health clinical standards, safety assessments, and care coordination
• Excellent communication and patient/family education abilities
• Comfortable with documentation systems and timely charting
• Ability to work independently in community/home settings while collaborating with team members
