Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Deliver personalized physical therapy services to patients recovering from surgery, falls, and other conditions in their homes
Develop individualized treatment plans that help patients regain strength, mobility, and independence
Collaborate with an interdisciplinary care team to ensure seamless, patient-centered support
Educate patients and families on safe exercises and recovery strategies
Document care accurately using the HomeCare/HomeBase EMR system

Qualifications

Current North Dakota Physical Therapy License
BLS certification
At least 1 year of physical therapy experience, including previous home health work
Valid driver's license and auto insurance
Comfortable traveling within a 60-mile radius of Valley City

Knowledge & Skills

Strong clinical skills in physical therapy and home health settings
Ability to manage a caseload of 5-7 patients per day
Familiarity with common diagnoses such as post-surgical recovery and fall prevention
Experience with HomeCare/HomeBase EMR preferred
Excellent communication and time-management abilities
