Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $70 per hour

Roles & Responsibilities

Deliver personalized physical therapy care to patients in the comfort of their homes
Perform Start of Care visits for orthopedic patients when nursing is not ordered
Help patients recover mobility and independence after joint replacements and other orthopedic procedures
Support geriatric patients managing multiple chronic conditions like CHF and COPD
Document visits accurately using the HomeCare HomeBase EMR system on your provided tablet
Collaborate with the intake RN and manager to coordinate care across Tehama County

Qualifications

Current Physical Therapist license in California
BLS certification required
Home Health experience preferred but will consider candidates with less than 2 years if they have Home Health background
Ability to complete approximately 5 patient visits per day
Comfortable traveling within Tehama County with an average of 40 miles daily

Knowledge & Skills

Physical Therapy evaluation and treatment planning
Home Health documentation and compliance
Orthopedic rehabilitation techniques
Geriatric care with multiple comorbidities
EMR proficiency with HomeCare HomeBase
Independent decision-making in home settings
