Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 4:00 PM or 9:00 AM - 5:00 PM, weekends as needed

Hours per Week: 40

Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Deliver compassionate home health therapy to patients in their own homes, making a real difference in their daily lives
Develop personalized treatment plans that empower patients to regain independence and mobility
Collaborate with interdisciplinary teams to ensure seamless, holistic care
Educate patients and families on exercises and strategies for long-term success
Monitor progress and adjust therapies to celebrate every milestone

Qualifications

Active Physical Therapy license in good standing
BLS certification
Experience in home health preferred
Epic experience a plus, but not required
Commitment to patient-centered care with a positive attitude

Education

Master's or Doctorate degree in Physical Therapy from an accredited program

Knowledge & Skills

Strong clinical assessment and treatment skills
Excellent communication for building patient rapport
Proficiency in home health documentation
Adaptability to varied home environments
Familiarity with Epic systems is advantageous
