Roles & Responsibilities

• Perform thorough patient evaluations and create individualized OT plans of care
• Conduct revisits and adjust treatment plans based on patient progress and goals
• Document all visits accurately and promptly in Cerner to ensure continuity of care
• Coordinate with home health supervisors for mileage/time approvals and paperwork
• Provide safe, evidence-based interventions in patients’ home environments
• Communicate clearly with interdisciplinary teams and patients’ families to optimize outcomes
• Maintain professional standards, certifications, and required licensure documentation

Qualifications

• Active CA OT license required
• Minimum 2 years experience in a healthcare setting
• BLS certification required
• Active NPI number required
• Valid driver’s license and proof of automobile insurance required for home health
• Two supervisory references at submittal (one within 12 months, one within 3 years)
• Cerner charting experience preferred
• COTA considered if no OT applicants and with home health experience

Knowledge & Skills

• Strong clinical skills in occupational therapy evaluation and intervention
• Home health workflow knowledge and comfort working in patient homes
• Proficient documentation skills in Cerner or similar EMR systems
• Ability to manage time and travel logistics across a coverage area (NE Sacramento into Folsom)
• Familiarity with GSA mileage billing practices and accurate timesheet submission
• Excellent communication and patient/family education skills
• Problem-solving and ability to adapt care plans to home environments
