Roles & Responsibilities

• Deliver safe, compassionate patient care, focusing on comfort and clinical needs
• Administer medications and treatments accurately and document thoroughly
• Perform thorough assessments and report changes to the care team promptly
• Collaborate with nurses, providers, and support staff to coordinate care plans
• Maintain clear, timely documentation in the patient record
• Support patient education and family communication with empathy and clarity
• Participate in quality improvement and safety initiatives to enhance outcomes

Qualifications

• Valid Licensed Practical Nurse (LPN) license — LPN credentials required; Registered Nurse (RN) candidates may also be considered where applicable
• Current Basic Life Support (BLS) certification required; ACLS preferred where applicable
• Comfortable with medication administration, IV care per facility policy, and standard nursing procedures
• Strong communication skills and ability to work as part of a multidisciplinary team
• Professional, reliable, and committed to patient-centered care

Education

• Graduate of an accredited practical nursing program (LPN) or equivalent nursing program

Knowledge & Skills

• Skilled in patient assessment and monitoring
• Knowledgeable in safe medication administration and documentation
• Proficient with basic charting and electronic health record systems
• Ability to prioritize care, manage time effectively, and respond to changing patient needs
• Good interpersonal skills for patient/family education and team collaboration
