Contract Type: Per-Diem

Shift Type: 8-hour shift; specific start/end times and scheduled days to be confirmed with the facility

Hours per Week: 40

Candidate Pay Rate: $34 per hour

Roles & Responsibilities

Provide direct, compassionate nursing care to residents and patients according to care plans
Administer medications and treatments safely and accurately
Monitor and record patient conditions, reporting any changes to the RN or supervisor
Assist with ADLs including bathing, dressing and mobility support when needed
Communicate clearly with families and the care team to coordinate excellent care
Maintain accurate documentation and comply with facility policies and regulatory standards

Qualifications

Current and active LPN license in Pennsylvania
Completion of an accredited practical nursing program
Basic Life Support (BLS) or CPR certification preferred or able to obtain
Strong clinical assessment and medication administration skills
Excellent communication and teamwork abilities
Reliable, professional, and committed to patient-centered care

Education

Graduate of an accredited Practical Nursing program
Active LPN license required

Knowledge & Skills

Knowledge of standard nursing procedures and documentation
Familiarity with medication administration and safety protocols
Ability to perform basic patient assessments and report findings
Good time management and organizational skills
Comfort working in long term care or similar care settings
Patient-focused approach with strong interpersonal skills
