Shift Type: Day shift, 7:00 AM - 3:30 PM, includes every other weekend

Hours per Week: 40

Candidate Pay Rate: $32 per hour

Roles & Responsibilities

Provide compassionate direct patient care under the supervision of RNs and physicians
Administer medications and treatments accurately and safely
Monitor patient vital signs and report changes promptly
Assist patients with daily living activities to promote independence and dignity
Collaborate with interdisciplinary team to create and implement care plans
Document patient care and progress in electronic health records

Qualifications

Valid LPN license in Vermont or compact state
Current BLS/CPR certification
Minimum 1 year of experience in skilled nursing or rehabilitation setting
Ability to work every other weekend and reliable transportation required

Education

Graduate of an accredited Licensed Practical Nursing program

Knowledge & Skills

Medication administration and management
Wound care and basic life support
Patient assessment and vital signs monitoring
Effective communication and teamwork
Electronic health record documentation
