Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM with on-call rotation

Hours per Week: 40

Candidate Pay Rate: $60 per hour

Roles & Responsibilities

Provide skilled nursing and case management for patients in their homes or at our in-patient Hospice Center
Assess, plan, and implement care using the nursing process and an interdisciplinary team approach
Oversee care provided by Home Health/Hospice Aides and CNAs
Ensure continuity of care through effective communication and case conferences
Maintain accurate patient records in compliance with OASIS requirements
Participate in on-call rotation and quality improvement activities

Qualifications

Current RN license in the State of Montana
Valid driver's license and reliable transportation
Minimum 2 years of recent acute care, home health, or hospice experience
BSN preferred

Education

Associate's Degree in Nursing (ADN) required
Bachelor's Degree in Nursing (BSN) preferred
