Candidate Pay Rate: $28 per hour

Roles & Responsibilities
- Provide compassionate, hands-on nursing care to patients in the Uptown Health/Rehab area
- Administer medications and treatments accurately and document per policy
- Monitor patient status, report changes to the care team, and assist with care planning
- Perform wound care, hygiene assistance, and other essential nursing procedures
- Communicate clearly with patients, families, and interdisciplinary team members
- Maintain accurate and timely electronic medical records and shift reports
Qualifications
- Active LPN license in good standing (required)
- BLS/CPR certification (or willingness to obtain upon hire)
- Comfortable working 12+ hour day shifts and managing a diverse caseload
- Strong communication skills and a collaborative attitude
- Previous experience in rehab, outpatient, or acute care settings preferred
Education
- Graduate of an accredited practical nursing program (LPN)
- High school diploma or GED required; additional nursing education a plus
Knowledge & Skills
- Skilled in medication administration and basic nursing procedures
- Patient assessment and monitoring
- Familiarity with infection control and safety protocols
- Proficient with electronic medical records and documentation best practices
- Excellent time management, prioritization, and teamwork abilities
