Roles & Responsibilities

• Provide compassionate, hands-on nursing care to assigned patients
• Administer medications and treatments safely and accurately
• Perform routine assessments and monitor patient status closely
• Document patient care and communicate changes promptly in the medical record
• Collaborate with RNs, providers, and the interdisciplinary team to optimize outcomes
• Support patient and family education with clear, empathetic communication
• Assist with mobility, ADLs, and basic procedures to promote recovery and comfort

Qualifications

• Active LPN or RN license in good standing (state licensure required)
• Current BLS (Basic Life Support) certification; ACLS or other certifications as required by area
• Clinical experience in acute care or rehab settings preferred
• Comfort with medication administration, patient assessments, and documentation
• Strong communication skills and ability to work as part of a collaborative care team

Education

• High school diploma or GED required
• Completion of an accredited practical nursing program (LPN) or nursing program for RNs

Knowledge & Skills

• Patient assessment and vital sign monitoring
• Medication administration and basic IV/PHARM knowledge appropriate to scope
• Wound care and dressing changes
• Charting and electronic medical record (EMR) documentation
• Time management, prioritization, and dependable clinical judgment
• Effective interpersonal skills with patients, families, and colleagues
