Roles & Responsibilities

• Provide compassionate, hands-on care to residents, assisting with daily living activities
• Take and record vital signs accurately and report changes promptly to nursing staff
• Assist with safe transfers, mobility and positioning to promote comfort and independence
• Maintain a clean, safe and respectful environment for residents
• Document care activities clearly and timely in resident records
• Collaborate with nurses, therapists and the care team to deliver person-centered care
• Follow infection control protocols and facility policies to keep residents safe

Qualifications

• Current Certified Nursing Assistant (CNA) certification (Pennsylvania preferred or eligible)
• Completion of an approved CNA training program
• Basic Life Support (BLS/CPR) certification preferred or able to obtain
• Current TB test, background check and immunizations as required by facility
• Strong communication skills and a caring, patient-focused attitude
• Ability to lift/assist residents and perform physical aspects of care

Education

• High school diploma or equivalent preferred
• Completion of an approved CNA training program required

Knowledge & Skills

• Proficiency with resident activities of daily living (ADLs) and mobility assistance
• Accurate vital signs measurement and basic clinical observation
• Knowledge of infection prevention and control practices
• Comfortable with charting/documentation and following care plans
• Teamwork, time management and strong interpersonal skills
• Ability to follow directions and escalate concerns appropriately
