Roles & Responsibilities
- Provide compassionate hands-on nursing care to residents in accordance with care plans
- Administer medications and treatments accurately and safely under LPN scope
- Monitor and document vital signs, responses to interventions, and changes in condition
- Collaborate with RNs, CNAs, physicians, and interdisciplinary team members to ensure continuity of care
- Assist residents with activities of daily living while promoting independence and dignity
- Maintain accurate, timely documentation in the facility charting/EMR system
- Support infection control practices and a safe, clean environment for residents
- Communicate clearly and respectfully with families to keep them informed and involved
Qualifications
- Active Licensed Practical Nurse (LPN) license — Maine licensure preferred or eligible to obtain
- Graduation from an accredited practical nursing program
- Current CPR/BLS certification (or willingness to obtain prior to start)
- Prior long-term care or skilled nursing experience preferred but new grads welcome with strong clinical skills
- Strong communication skills and the ability to work well in a team-based setting
- Ability to pass facility background checks and meet onboarding requirements
Education
- Graduate of an accredited Practical Nursing program (LPN diploma or equivalent)
Knowledge & Skills
- Solid knowledge of medication administration principles and documentation
- Comfortable with wound care, trach care, and common LPN tasks in a long-term care setting
- Familiarity with charting in electronic medical records and basic computer skills
- Good clinical assessment skills and the ability to escalate concerns appropriately
- Time management and prioritization during busy shift hours
- Compassionate patient-centered approach and strong teamwork orientation
