Roles & Responsibilities

• Provide compassionate hands-on nursing care to residents in accordance with care plans
• Administer medications and treatments accurately and safely under LPN scope
• Monitor and document vital signs, responses to interventions, and changes in condition
• Collaborate with RNs, CNAs, physicians, and interdisciplinary team members to ensure continuity of care
• Assist residents with activities of daily living while promoting independence and dignity
• Maintain accurate, timely documentation in the facility charting/EMR system
• Support infection control practices and a safe, clean environment for residents
• Communicate clearly and respectfully with families to keep them informed and involved

Qualifications

• Active Licensed Practical Nurse (LPN) license — Maine licensure preferred or eligible to obtain
• Graduation from an accredited practical nursing program
• Current CPR/BLS certification (or willingness to obtain prior to start)
• Prior long-term care or skilled nursing experience preferred but new grads welcome with strong clinical skills
• Strong communication skills and the ability to work well in a team-based setting
• Ability to pass facility background checks and meet onboarding requirements

Education

• Graduate of an accredited Practical Nursing program (LPN diploma or equivalent)

Knowledge & Skills

• Solid knowledge of medication administration principles and documentation
• Comfortable with wound care, trach care, and common LPN tasks in a long-term care setting
• Familiarity with charting in electronic medical records and basic computer skills
• Good clinical assessment skills and the ability to escalate concerns appropriately
• Time management and prioritization during busy shift hours
• Compassionate patient-centered approach and strong teamwork orientation
