Roles & Responsibilities
- Provide compassionate, skilled nursing care to long-term care participants, focusing on safety and dignity
- Perform thorough assessments and monitor changes in condition, intervening as needed
- Administer medications and treatments accurately and document in the medical record
- Develop and update individualized care plans in collaboration with the care team
- Educate participants and families about health conditions, treatments, and preventive care
- Communicate clearly with interdisciplinary partners to ensure coordinated, high-quality care
- Maintain regulatory and documentation standards, including timely charting and reporting
Qualifications
- Active RN licensure (Registered Nurse) — ability to practice in Louisiana is preferred
- Experience or interest in long-term care, geriatric nursing, or community-based care
- Current BLS (Basic Life Support) certification required or obtainable upon hire
- Required health documentation: MMR, Hepatitis B, Varicella, Tdap, Physical/Health Statement
- Required vaccinations and screens: COVID-19 vaccine, annual influenza vaccine, and Trinity 10 Panel drug screen
- Strong clinical judgment, excellent communication skills, and a patient-centered approach
Education
- Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) — RN licensure required
Knowledge & Skills
- Skilled in clinical assessment and early recognition of changes in condition
- Proficient with medication administration and care plan implementation
- Comfortable working within interdisciplinary teams and coordinating care
- Good documentation practices and familiarity with electronic health records
- Ability to prioritize tasks, manage time, and handle a varied caseload
- Empathy, patience, and excellent interpersonal skills for working with older adults
