Shift Type: Per Diem / PRN — variable shifts as scheduled (day, evening, or night); exact days and start/end times assigned by facility based on patient needs

Roles & Responsibilities
- Provide compassionate, resident-centered nursing care in a long-term care setting
- Perform thorough assessments and develop individualized care plans
- Administer medications and treatments safely and accurately
- Document clinical findings and charting clearly and timely in the resident record
- Collaborate with interdisciplinary team members to coordinate care and transitions
- Monitor resident condition and escalate changes to the care team as needed
- Support infection prevention practices and maintain required health screenings
- Participate in competency testing and ongoing education to maintain high standards of care
Qualifications
- Active Registered Nurse (RN) licensure with ability to verify license and certifications
- Current BLS certification (required)
- Experience or comfort working in long-term care settings preferred
- Completion of required Trinity specialty competency testing and drug screening as part of onboarding
- Up-to-date occupational health requirements: fit test, 2-step PPD or IGRA, MMR, Hep B, Varicella, Tdap, COVID-19 and Influenza vaccine or appropriate exemptions
- Ability to pass respirator evaluation and color vision testing as required by the facility
Knowledge & Skills
- Strong clinical assessment and medication administration skills
- Knowledge of long-term care resident needs and chronic condition management
- Infection control practices and ability to follow facility protocols
- Clear, compassionate communication with residents, families, and team members
- Comfortable with electronic documentation and basic computer skills
- Ability to prioritize, multitask, and adapt in a fast-paced care environment
