Shift Type: Per diem (PD Pool) - varied shifts; days, evenings, nights and weekends as needed (schedule is flexible and assigned based on unit needs)

Roles & Responsibilities
- Provide compassionate, patient-centered nursing care to long-term care participants
- Perform thorough assessments and develop individualized care plans
- Administer medications and treatments safely and accurately
- Collaborate with interdisciplinary team members to coordinate care
- Document clinical findings and interventions clearly and promptly
- Participate in skills validation and competency checks (Trinity Skills Checklist)
- Support infection prevention practices and maintain a safe care environment
Qualifications
- Active Registered Nurse (RN) license (license/certificate verification required)
- BLS certification required
- Experience or interest in Long-Term Care (LTC) settings
- Successful completion of Trinity Skills Checklist and required onboarding
- Required health clearances and screenings (Fit Test, 2-step PPD or IGRA, MMR, Hepatitis B, Varicella, Tdap, COVID-19 vaccine, Influenza vaccine, Trinity 10-panel drug screen)
- Ability to work flexible per-diem shifts and adapt to changing unit needs
Knowledge & Skills
- Strong clinical assessment and medication administration skills
- Familiarity with long-term care best practices and resident-centered care
- Competency with documentation and electronic health records
- Infection control and safe work practices
- Effective communication and teamwork in interdisciplinary settings
- Comfort completing skills checklists and competency-based evaluations
