Roles & Responsibilities

• Assess speech, language, cognitive-communication, and swallowing function for SNF residents
• Develop and implement individualized treatment plans that focus on meaningful functional gains
• Document evaluations and progress notes in accordance with CRG policies and facility standards
• Collaborate with nurses, PT/OT, CNAs, and family members to coordinate comprehensive care
• Provide caregiver and family education to support carryover and discharge planning
• Maintain productivity expectations and prioritize efficient, high-quality care (90% productivity noted)
• Adhere to facility dress code (black scrubs) and be on-site to receive guaranteed hours
• Communicate clearly and professionally with the interdisciplinary team and facility leadership

Qualifications

• 1-3 years of Speech-Language Pathology experience (as indicated in the order)
• Valid license or eligibility for licensure to practice SLP in New Mexico (state licensure required for practice)
• ASHA CCC-SLP preferred where applicable
• Experience working in a skilled nursing facility or post-acute/rehab setting preferred
• Current professional clearances and immunizations per facility requirements
• Strong documentation skills and ability to meet productivity expectations

Knowledge & Skills

• Skilled in assessment and treatment of dysphagia and cognitive-communication disorders
• Proficient with SNF documentation standards and regulatory requirements
• Ability to create goal-directed, patient-centered therapy plans
• Strong interdisciplinary communication and care coordination skills
• Comfortable working in a fast-paced rehab environment and managing caseloads
• Familiarity with productivity metrics and efficient clinical workflows
