Roles & Responsibilities

• Conduct thorough SLP evaluations for residents in a skilled nursing facility
• Provide daily treatments focused on improving communication and swallowing function
• Coordinate and manage safe, timely discharges for residents
• Document clinical care accurately and follow CRG policies & procedures
• Collaborate with an interdisciplinary therapy team to create person-centered care plans
• Maintain productivity targets and contribute to a high-performing therapy program

Qualifications

• 1-3 years of clinical experience (as listed in the job posting)
• Prior SNF experience strongly preferred
• Ability to be present on-site in order to receive guaranteed hours
• Willingness to follow CRG policies & procedures and meet a 90% productivity requirement
• Comfort with facility dress code: black scrubs
• Available (or willing) to start by the requested date of 2025-12-01

Knowledge & Skills

• Skilled in SLP evaluations, daily treatment planning and execution
• Experience managing discharges in a skilled nursing facility
• Familiarity with SNF workflows and documentation practices
• Ability to collaborate with a multidisciplinary therapy team
• Productivity and time-management skills to meet facility targets
