Hours per Week: 40

Candidate Pay Rate: $53 per hour

Roles & Responsibilities

Coordinate seamless patient care transitions to optimize outcomes
Master complex discharge planning for safe patient returns home
Navigate Meditech and Midas systems with confidence
Collaborate with RN and SW case managers to support patient ratios
Advocate for patients ensuring they receive the best possible care

Qualifications

Active RN license in good standing
At least 1 year of case management experience
Proficiency in Midas and Meditech required
Experience with complex discharge planning preferred

Education

RN degree from accredited nursing program

Knowledge & Skills

Expertise in RN Case Management
Strong skills in Meditech and Midas EMR systems
Ability to handle complex discharge planning
Team collaboration with social workers and staff
