Contract Type: Contract

Shift Type: Monday-Friday, 7:30 AM - 4:00 PM or 8:00 AM - 4:30 PM, up to every other weekend

Hours per Week: 40

Candidate Pay Rate: $71 per hour

Roles & Responsibilities

Lead meaningful goals-of-care conversations with patients and families
Manage a daily caseload of 5-6 new consults and 3-4 follow-ups
Review patient charts and assess needs across the hospital
Educate patients on disease processes, advance directives, and POLSTs
Facilitate family meetings and provide compassionate end-of-life support
Collaborate during daily rounds with physicians and the palliative team

Qualifications

Active California RN license
BLS certification required
2-5 years recent palliative or hospice experience
Strong acute care clinical thinking and communication skills
ELNEC certification preferred

Knowledge & Skills

Palliative care screening and consultation workflow
Cerner EMR documentation
Goals-of-care discussions and family facilitation
Advance directives and POLST completion
Compassionate end-of-life communication
