Roles & Responsibilities

• Assess patients to determine mental, physical, and spiritual needs and document findings clearly
• Develop, implement and evaluate individualized plans of care in collaboration with the multidisciplinary team
• Provide clear explanations and instructions to patients and their families to support recovery
• Use nursing clinical judgment to promptly recognize and report changes in patient condition
• Maintain complete, accurate medical records and meet Core Measure responsibilities
• Contribute to a healthy workplace by managing conflict professionally and communicating effectively
• Participate in continuing education to keep skills sharp and evidence-based

Qualifications

• Current Illinois Registered Nurse (RN) license
• Current CPR/BLS certification
• Experience caring for inpatient rehabilitation patients; comfort with neuro populations is a strong plus
• Stroke certification is preferred but not required
• Willingness to work 12-hour night shifts and every third weekend (Friday & Saturday nights)
• Strong clinical judgment, assessment skills, and effective communication

Knowledge & Skills

• Patient assessment and clinical judgment
• Care planning and interdisciplinary collaboration
• Inpatient rehabilitation practices and neurorehabilitation awareness
• Medical documentation and maintaining accurate patient records
• BLS/CPR and acute patient monitoring
• Familiarity with stroke care best practices (preferred)
