Roles & Responsibilities

• Provide compassionate, patient-centered hospice nursing in homes, assisted living and nursing facilities
• Perform thorough assessments and lead symptom management to maximize patient comfort
• Serve as the Primary RN for an assigned caseload and coordinate individualized care plans
• Educate patients and caregivers so families feel confident and supported
• Lead and collaborate with the interdisciplinary team to ensure cohesive care
• Document clinical findings, care plans and communication clearly and promptly
• Respond to occasional on-call needs and provide back-up coverage as needed
• Demonstrate leadership in clinical decision-making and critical thinking for complex cases

Qualifications

• Active Registered Nurse (RN) licensure (RN) — bring your clinical expertise
• Experience in hospice care, symptom management and patient assessment
• Strong critical thinking and clinical assessment skills
• Ability to manage a caseload and lead interdisciplinary team discussions
• Excellent communication skills for patient and caregiver education
• Commitment to compassionate, family-centered end-of-life care
