Roles & Responsibilities

• Perform thorough physical therapy evaluations and develop individualized treatment plans that help residents regain function
• Deliver skilled therapeutic interventions to improve mobility, strength, balance and safety
• Document assessments, plans and progress accurately and timely in the facility’s medical record
• Collaborate with nurses, OT, SLP and restorative staff to provide coordinated, resident-centered care
• Educate residents and caregivers on home exercise programs and fall prevention strategies
• Participate in care planning and discharge planning to maximize functional outcomes
• Adhere to facility policies, regulatory requirements and best-practice standards

Qualifications

• DPT or degree in Physical Therapy (PT) — credentials consistent with PT/DPT listed in the job
• Licensed or eligible for licensure as a Physical Therapist in Colorado
• Comfortable working in a Skilled Nursing Facility (SNF) practice setting
• Open to clinicians at all experience levels — any experience accepted, including new grads
• Strong documentation skills and ability to work collaboratively with interdisciplinary teams

Education

• DPT (Doctor of Physical Therapy) or equivalent degree in Physical Therapy

Knowledge & Skills

• Solid knowledge of physical therapy principles and interventions for older adults
• Experience (or familiarity) with SNF workflows, restorative nursing and mobility programs
• Skilled in gait training, transfers, therapeutic exercise and balance training
• Clear clinical documentation and treatment planning skills
• Good communication and teamwork — working well with residents, families and staff
