Roles & Responsibilities

• Evaluate and treat residents in skilled nursing and post-acute settings to maximize functional outcomes
• Develop and implement individualized treatment plans focused on safe, meaningful progress
• Collaborate closely with an interdisciplinary team to coordinate seamless care
• Document accurately and timely in accordance with facility and regulatory standards
• Participate in care conferences and contribute clinical insight to care plans
• Engage in ongoing quality improvement efforts to elevate patient outcomes and service

Qualifications

• Active California Physical Therapist license (required)
• Strong clinical skills with the ability to evaluate and treat post-acute/skilled nursing residents
• Excellent communication skills for patient interactions and team collaboration
• Skilled nursing or post-acute experience preferred
• Dependable, flexible, and team-oriented approach to care delivery
• Commitment to high-quality, compassionate patient-centered care
