Roles & Responsibilities

Conduct thorough psychosocial assessments to understand patients' strengths, needs, and goals
Develop individualized treatment plans that are practical, evidence-based, and person-centered
Provide evidence-based therapeutic interventions across the lifespan, addressing depression, anxiety, trauma, substance use, and adjustment disorders
Coordinate care with primary care providers, nurses, and other behavioral health professionals to ensure whole-person care
Document clinical encounters clearly and timely, maintaining high standards for clinical recordkeeping
Advocate for patients in rural and medically underserved settings to improve access to services and community supports
Participate in integrated care team meetings and contribute clinical insights to improve patient outcomes

Qualifications

Master's degree in Social Work from an accredited program (required)
Active LISW license in Iowa (required)
Minimum of 2 years of clinical experience preferred
Experience in rural or medically underserved settings is a plus
Strong documentation, interpersonal, and organizational skills
Comfort working as part of an integrated care team and coordinating with primary care

Education

Master's degree in Social Work from an accredited program

Trauma-informed care and substance use treatment approaches
Care coordination within integrated primary care teams
Clinical documentation best practices and regulatory awareness
