Roles & Responsibilities

• Evaluate patient mobility, function, and pain to develop individualized rehabilitation plans
• Deliver skilled therapeutic interventions including gait training, therapeutic exercise, and manual therapy
• Collaborate with interdisciplinary team members to coordinate patient-centered care and discharge planning
• Document patient progress clearly and timely in the medical record to support outcomes and continuity
• Educate patients and families on home exercise programs and strategies to maximize recovery
• Adapt treatment plans based on clinical response and changing patient needs to drive meaningful outcomes

Qualifications

• Current Physical Therapist license (active and in good standing) — state licensure required for practice
• Degree in Physical Therapy (DPT, MPT, or equivalent) from an accredited program
• Basic Life Support (BLS) certification required; additional certifications encouraged
• Strong clinical assessment and documentation skills with a patient-first mindset
• Comfort working in a fast-paced rehabilitation environment; travel experience is a plus
• Professional communication skills and ability to collaborate effectively with interdisciplinary teams

Education

• DPT preferred; MPT or BS in Physical Therapy acceptable where applicable

Knowledge & Skills

• Skilled in patient assessment, therapeutic exercise, and mobility/gait training
• Proficient with treatment planning, outcome measurement, and clinical documentation
• Knowledge of rehabilitation approaches for post-acute and outpatient populations
• Ability to instruct patients and caregivers in safe home exercise programs and fall prevention
• Strong interpersonal skills for building therapeutic rapport and working within a multi-disciplinary team
