Roles & Responsibilities

• Provide compassionate, high-quality patient care across the multidisciplinary center
• Administer medications and treatments accurately and safely
• Monitor and record vital signs and communicate changes to the care team
• Document patient care clearly and timely in the electronic medical record
• Assist with admissions, discharges, and safe patient transfers
• Collaborate with nurses, therapists, and clinicians to create and follow care plans
• Support a positive patient experience by educating patients and families

Qualifications

• Required: Current LPN license in the state of practice (New Hampshire)
• Comfortable working the listed schedule: Sunday, Monday, Tuesday, Wednesday, Friday, 7:00 AM - 3:30 PM
• Preferred: BLS/CPR certification or willingness to obtain upon hire
• Ability to pass facility onboarding, background check, and required health screenings
• Strong communication skills and a team-first attitude

Education

• Diploma or certificate in Practical Nursing (LPN/LVN program)
• High school diploma or equivalent

Knowledge & Skills

• Medication administration and documentation best practices
• Basic nursing assessments including vitals and focused assessments
• Familiarity with wound care and common nursing procedures
• Competence with electronic medical records and charting
• Ability to prioritize tasks in a fast-paced clinical environment
• Collaborative skills to work within a multidisciplinary care team
