Roles & Responsibilities
- Provide compassionate physical therapy services across four rural counties
- Conduct OASIS assessments and Start of Care evaluations for new patients
- Help elderly patients recover from falls and fractures with personalized treatment plans
- Manage a manageable caseload of 4-6 patient visits daily using a point-based system
- Document visits and track time efficiently with provided tablet technology
Qualifications
- Licensed Physical Therapist in Indiana
- Experience with OASIS assessments and Start of Care documentation
- Comfortable working independently in rural community settings
- Strong interpersonal skills for building trust with elderly patients
Knowledge & Skills
- OASIS assessment expertise
- Start of Care evaluation skills
- Rural healthcare experience preferred
- Tablet-based documentation proficiency
- Geriatric rehabilitation knowledge
