Physical Therapist - Home Health
Location: Red Bluff, California

Contract Type: Travel Contract
Shift Type: Monday-Friday, 8:00 AM - 5:00 PM
Hours per Week: 40
Candidate Pay Rate: $70 per hour

Roles & Responsibilities
- Provide compassionate physical therapy services to patients in their homes throughout Tehama County
- Conduct Start of Care visits especially for orthopedic patients when nursing is not ordered
- Develop personalized treatment plans for geriatric patients with multiple comorbidities including CHF and COPD
- Complete approximately 5 meaningful patient visits per day using the HCHB EMR system
- Travel within the service area with mileage reimbursement provided
Qualifications
- Current Physical Therapy license in California
- BLS certification required
- Home Health experience preferred but will consider candidates with less than 2 years experience who have Home Health background
- Strong assessment and documentation skills
Knowledge & Skills
- Physical Therapy expertise in home health settings
- Experience with orthopedic and joint replacement patients
- Comfort working with geriatric populations and complex medical conditions
- Proficiency with electronic medical records, preferably HCHB
- Excellent time management and independent decision-making abilities
