Roles & Responsibilities
- Deliver compassionate, hands-on care to residents in a skilled nursing setting
- Administer medications and treatments accurately while monitoring patient responses
- Collaborate with the interdisciplinary team to create and update individualized care plans
- Educate patients and families on health management and recovery strategies
- Document all care activities thoroughly to ensure continuity and compliance
Qualifications
- Active LPN or RN license in New Hampshire or compact state
- Current BLS certification
- Minimum of one year recent experience in long-term care or skilled nursing preferred
- Strong assessment and communication skills
