Roles & Responsibilities

Provide patient-centered physical therapy care in home settings, improving mobility and independence
Assess functional status, develop individualized treatment plans, and set measurable goals
Document patient progress clearly and timely in the EMR to support continuity of care
Educate patients and caregivers on exercises, fall prevention, and home safety strategies
Collaborate with interdisciplinary team members to coordinate holistic care plans
Manage caseload efficiently, schedule visits, and ensure timely follow-up

Qualifications

Current state Physical Therapist (PT) license (required)
Active CPR/BLS certification or willingness to obtain
Home health or outpatient experience preferred but enthusiastic new grads welcome
Strong clinical reasoning with ability to develop practical home-based programs
Reliable transportation and a clean driving record for travel between patient homes

Education

Degree in Physical Therapy (DPT, MPT or equivalent)

Knowledge & Skills

Clinical assessment and mobility improvement techniques
Home safety evaluation and adaptive equipment recommendations
Clear, compassionate patient and caregiver education
Good documentation practices and comfort with electronic medical records
Time management and route planning for efficient home visits
Familiarity with Medicare and home health regulatory basics is a plus
