Roles & Responsibilities

• Provide skilled physical therapy to patients in their homes, delivering compassionate, evidence-based care
• Perform comprehensive evaluations and develop individualized treatment plans that promote independence
• Conduct regular patient visits, track progress, and update goals with measurable outcomes
• Document all visits and clinical notes accurately in HCHB to ensure continuity of care
• Coordinate with the interdisciplinary team, families and caregivers to optimize patient outcomes
• Educate patients and caregivers on home exercise programs, safety, and fall prevention
• Manage a caseload of approximately 5–6 patients per day while prioritizing patient-centered care
• Maintain reliable transportation for patient visits and represent the agency professionally in the community

Qualifications

• Minimum 2 years of relevant experience (as listed in the job posting)
• Demonstrated home health experience for therapy positions (explicitly required)
• Proficiency with patient visits and documentation workflows
• Comfort using HCHB documentation system (explicitly noted in posting)
• Reliable vehicle for conducting patient home visits (posting requests this)

Knowledge & Skills

• Skilled in home health therapy assessment and treatment planning
• Strong documentation skills and familiarity with HCHB
• Excellent communication and patient/caregiver education skills
